Eh, you'll certainly be getting some varying opinions on this subject.
There's been a few posts of this nature; and I'm sure this one will go the same way.
I personally think all IPTV providers should offer a form of BTC payment; as this whole "Paypal" thing seems convenient on the surface, but it opens up some risky possibilities as you pointed out. I've purchased a sub once in the past with Paypal, and that was almost a year ago now. No scary letters or knocks on the door, yet! Paypal account still in perfect standing.
Back to your original question though, there's not many providers I've found what-so-ever that offer it. Over on Bitcoinforums.org, you will find a seller or two offering IPTV for BTC, but they are not popular services (can be a good thing), nor do they integrate with popular tv guide add-ons that easily (may not bother you). It really depends on the type of service you're looking for, and the channels you expect to get.
I'm still waiting for the day a reliable provider such as NFPS (Stalker), NTV, etc. begin offering payment via Bitcoin.
I've gone as far as asking the question on a couple "IPTV" oriented Facebook groups, and was flamed to no end for even bringing up "anonymous payments". As if the services we're paying for aren't providing essentially "stolen content", why in the world wouldn't providers want anonymity for not only themselves, but their users as well?

Yes, they really all should. One obvious problem is that if you actually need your paypal account, you could lose it after something like this.
Two, you have the identity issue. I'm not so sure on that, but I remember in the days before iptv, this was a huge deal with dave and charlie, and back then people weren't even paying any regular "subscriptions", but instead just buying hardware over the phone or internet with credit card.
