I mean, do they have sources that have paid for channels, and those sources then just upload to IPTV providers, who then in return, stream it forward to end users?

Either they are a reseller, or they provide public links in a playlist.
Well, it's easy. Companies rent a server, broadcast their stuff to the public, release the RTMP/M3U8 links to sites like cCloud and derivates, profit. :D
