I think Streamcenter.tv (new version of Streamtvbox) has a Sport Center that is likely sourced from Smoothstreams. They also have a twitter page too.

Vader Streams meets that criteria but I'm not sure if they're accepting new members at this time. Perhaps a reseller of their service can comment on that.
