There are a few high level providers who outsource their channel access to most, if not all of the services we see here. They get their channels by buying a ton of valid subscriptions through Directv, Sky... then set up data centers that do nothing but capture the feeds and upload them to their servers. Takes a lot of hardware and bandwidth to this. From there they sell the access to the resellers. Along with securing streams through the valid subs, i've also heard that they get them through hacked encrypted signals, legitimately streamed channels like ESPN, and captures from individuals who stream some channels. They gather them all and package them into one place on their servers for redistribution to the resellers.

There are 2-3 sources for UK, 2-3 for USA, and a few more for each big Europe country.
