If it's MAC Address based (not sure what that's called), you can use iSTB which is $14. Couldn't find another option
If it's m3u based, I've used GSE which is $5.
GSE is shit full off adds. Try cloud stream or Xtream iptv they are free.

Its not very good though and you'll miss out on a lot iptv has to offer, depending what app you use (very few to chose from)
