First off, a MAG box is a dedicated IPTV box, if you decide to use this, then your provider will request info from the box to register it on their system and give the box access.
You will need to provide the MAC address of the MAG box if it applies to you. If you don't have a MAG or plan on using one, then ignore it. If the provider doesn't speak English as their native language (or is commonly lazy), their site might have merged "MAG MAC address" into just "MAG address".
Some providers, even if you subscribe to a multiple connection plan, will automatically put you to one connection if using a box.
